black screen after an "apply image" task sequence reboot

anyone have any thoughts on why i am getting a black screen after the "apply image" task sequence reboot?

Hello Bill,
Did you see it on all Macs? What OS X version you are deploying?

not on all, however it is widespread.
OS X El Capitan

This was fixed in PMM 5.0 Update1. You have to re-build Boot Image after upgrade.
