where is the key number for REFUND

I am desperately trying to fill the form to receive a refund but the page keeps telling me my key number is not valid.
Is the key number my activation key? If not, where can I find it?

Hello ezmerel2. Could you please specify where did you buy the Parallels product you wish to return and what is the form you are talking about? Web links will be much appreciated.
 
I was having the same problem with the form, so I just filled in the "order number" but not the key number (neither is marked as 'required'). The form was accepted.

I haven't gotten a reply yet, but at least I was able to submit my request.
